Support Worker

Our client is looking for a support worker to join their team

Duties of the Support Worker will include, but is not limited to:

Processing customer referrals for the floating support service including detailed risk assessments with the customer and evaluating their immediate and longer-term support needs

  • Supporting customers in both areas of the service which include resettlement and sustainment services
  • Types of support could consist of helping to set up customers' bills and liaising with utility companies around tariffs / charges, change addresses and add housing costs to UC claims or set up those claims for customers as well as support with all complex benefits issues including applications for PIP and limited capability for work and support customers throughout that process
  • You might also access grants and other charitable services for white goods and furniture, support to access mental and/or physical health services and substance misuse services as applicable
  • Sustainment is longer term support for those customers who need help with more complex issues and are at risk of losing their tenancy and becoming homeless this might be due to difficulties with budgeting / debts, poor physical or mental health, domestic abuse, substance misuse issues and / or anti-social behaviour
  • You will need to work with the local housing authority to make sure they are aware the customer has been assessed, accepted, and support has commenced
  • Setting up customers on the appropriate internal case management system and undertaking case management responsibilities
  • Making sure a formalised induction, support and an exit pathway plan is created in conjunction with the customer, within the first four weeks in service
  • Leading person centred and confidential support meetings with customers on your caseload in a setting which is safe

The successful candidate will have:

  • Full UK Driving Licence and access to a vehicle
  • Enhanced DBS check
  • GCSE Maths and English grade A-C (preferred)
  • Relevant experience in a support setting
